POLICIES

These policies are designed to support clear communication, effective care, and a smooth treatment experience.

 

Fees & Payment

The initial 90-minute comprehensive evaluation is $300, and follow-up appointments are $150 (up to 30 minutes).

 

Brownsburg Wellness is a self-pay practice and is not in network with any insurance plans. Payment is required at the time of service.

 

You may contact your insurance carrier to determine whether you are eligible for out-of-network reimbursement. A superbill can be provided upon request, if eligible.

 

Brownsburg Wellness does not accept Medicaid or Medicare. Patients are responsible for all services rendered and agree not to submit claims to Medicaid or Medicare for reimbursement.

 

Cancellation

Successful psychiatric care requires a commitment to regular follow-up. Please plan to attend all scheduled appointments.

 

A $100 fee will be charged for missed appointments or cancellations made without at least 24 hours’ notice. This time is reserved exclusively for you.

 

Clients who repeatedly miss or cancel appointments without appropriate notice may not be rescheduled.

 

Please notify the practice as soon as possible if you are unable to attend a scheduled appointment.

 

Communication

Communication via text message or standard email is not considered fully HIPAA-secure.

 

These methods are best used for brief, non-urgent matters such as scheduling, simple updates, or general questions.

More complex concerns, medication changes, or ongoing discussions may require an appointment to ensure appropriate care.

I strive to be accessible between appointments and aim to respond within 1 business day.

Medication

All prescription medications require regular appointments and ongoing monitoring.

​

Please ensure you have enough medication to last until your next appointment. If a refill is needed, allow up to one business day for processing.

​

Medication changes are typically addressed during scheduled appointments. Minor adjustments may be discussed, though an appointment may be required depending on the situation.

 

Please check with your pharmacy to confirm whether refills are available before contacting the practice.

 

Automated pharmacy fax refill requests are not monitored. Please contact the practice directly for refill requests or medication concerns.

Scope of Care

This practice provides outpatient psychiatric evaluation and medication management for mild to moderate mental health concerns and is best suited for individuals seeking focused care and a collaborative approach to treatment.

 

Patients requiring more intensive, frequent, or comprehensive services—including those with significant functional impairment or higher-acuity needs—may be referred to a higher level of care or to providers better equipped to meet those needs.

 

To ensure appropriate care, referrals may be made if a patient’s needs fall outside the scope of this practice or if treatment recommendations are not followed.

 

Administrative Documentation

Brownsburg Wellness does not complete FMLA, short-term disability (STD), long-term disability (LTD), or other third-party disability, return-to-work, or benefits-related documentation, including work requirement exemptions for state or federal assistance programs.

These determinations require a more comprehensive functional evaluation and fall outside the scope of this practice.

​

A copy of your records and/or a brief, objective treatment summary can be provided upon request. Work notes for time missed due to appointments are also available.

 

Confidentiality

Brownsburg Wellness respects your right to privacy and holds your confidentiality in the highest regard.

 

Information related to your care is maintained in accordance with federal (HIPAA) and state laws.

 

Non-Discrimination

Brownsburg Wellness does not discriminate on the basis of religion, race, gender, marital status, age, sexual orientation, national origin, prior incarceration, disability, or public assistance status.

 

Emergencies

This practice does not provide emergency services.

 

If you are experiencing a crisis, please call 911 or 988 (Suicide & Crisis Lifeline), or go to your nearest emergency room.
