POLICIES
FEES
The initial 90-minute comprehensive evaluation is $300, and all follow-up appointments are $150 (up to 30 minutes). As I operate a fee-for-service model, fees are due and collected at the time of service, and services will not be provided without payment. Accepted forms of payment are HSA/FSA cards and all major credit cards. I do not currently accept or process insurance claims; however, upon request, I can provide receipts that include all of the necessary information for patients to seek out-of-network reimbursement. I am unable to schedule clients with Medicaid or Medicare.
CANCELLATION REQUEST
Successful psychiatric services require a commitment on your part. You are responsible for attending all scheduled appointments. Reminder texts are provided as a courtesy only. A 24-hour notice is required to cancel or reschedule follow-up appointments, and a 48-hour notice is required for new intake appointments. This time has been reserved exclusively for you; therefore, the policy is to charge $100 for missed appointments or for cancellations without a 24-hour notice.
If you are unable to keep regularly scheduled appointments, I will not be able to continue to treat you.
MEDICATIONS
I am unable to make complete medication changes between appointments. If minor adjustments need to be made, please call to discuss this, however, an appointment may be necessary depending on individual circumstances.
All prescription medications require regular appointments and careful monitoring as part of your care. I make every effort to prescribe enough medication to last until your next appointment; therefore, please check with your pharmacy to verify that there are no refills/scripts on file prior to requesting one. I recommend calling the pharmacy to ask if there is a prescription on hold in their system.

I do not accept automated pharmacy fax refill requests/ notifications. If you need a refill or have any issues obtaining your medication, please call to discuss this.
COMMUNICATIONS
Communications sent via text message or standard email are not considered fully HIPAA-secure methods of communication. These methods are best used for scheduling or administrative purposes.

If you choose to communicate personal health information through text or email, please be aware that I cannot guarantee the privacy or security of these communications. By choosing to communicate in this way, you acknowledge and accept these risks and consent to the use of these communication methods.

DISABILITY/FMLA
As I treat patients with mild to moderate concerns, I am unable to complete FMLA, Short-Term Disability (STD), Long-Term Disability (LTD), or other third-party disability or return-to-work paperwork. The conditions that meet these requirements are outside the scope of my psychiatric practice and treatment focus. I can provide a copy of your records and/or a brief, objective treatment summary. I am also happy to provide notes for time missed due to appointments.
