The initial 60-minute evaluation is $240 and all follow-up appointments are $120 (up to 30 minutes). As I operate a fee-for-service model, fees are due and collected at the time of service, and services will not be provided without payment. Accepted forms of payment are HSA/FSA cards and all major credit cards. I do not currently accept or process insurance claims. Upon request, I can provide receipts that include all of the necessary information for patients to seek out of network reimbursement.
Successful psychiatric services require a commitment on your part. You are responsible for attending all scheduled appointments. Reminder texts are provided as a courtesy only. A 24 hour notice is required to cancel or reschedule an appointment. This time has been reserved exclusively for you, therefore the policy is to charge full cost of the appointment for all missed appointments or for cancellations without a 24 hour notice.
I am unable to make complete medication changes between appointments. If minor adjustments need to be made, please call to discuss this, however an appointment may be necessary depending on individual circumstances.
All prescription medications require regular appointments and careful monitoring as part of your care. I make every effort to prescribe enough medication to last until your next appointment. Please check with your pharmacy to verify that there are no refills/scripts on file before contacting me. I recommend calling the pharmacy to ask if there is a prescription on hold in their system.
If a medication needs to be sent in due to missing/rescheduling an appointment, there will be a fee of $25. If you are unable to keep regularly scheduled appointments, I will not be able to continue to treat you.
I do not accept automated pharmacy fax refill requests/notifications. If you need a refill or have any issues obtaining your medication, please call to discuss this.

Leave a message and I will return your call that day or the following day (during normal business hours M-F: 9am to 5pm). Communications sent via text or email are not considered HIPAA compliant methods of communication. I recommend using these methods of communication for discussing appointments. If you choose to send or request personal health information via these methods, I cannot guarantee full security/protection of your information, and you will be doing so at your own risk.
